Contribute by Check

Thank you for choosing to be an important part of Geisinger’s government relations efforts.  You can contribute by check by clicking the green “check contribution form” button below and printing the pdf.  Once the form has been printed and entirely filled out, please mail it and your contribution made by check to the address provided on the form.
